Pivot Table Definition - Schedule: Treatment Retention

Initial Data Set
Any appointment on the schedule where the
- Appointment Type has the "Procedure" box checked AND
- the appointment date (not create date) is in the date range AND
- the appointment is checked in AND
- the appointment is not deleted AND
- the patient is not deleted
For the Procedure appointments above, any appointment where the
- Appointment Type has the "Procedure" box checked AND
- the appointment date (not create date) is after the consult appointment date AND
- the appointment is not cancelled
- the appointment is not deleted

- # Patients = Count of procedure appointments

- % Booked = Not Booked divided by # Patients expressed as a %
